All 12-15 year olds must either have the written parental consent form completed (provided by your school), or have a parent/guardian present to provide verbal consent.
ID will be needed in the form of birth certificate, passport or medical card.
Please note: Aged 12-17 (if tested positive for COVID-19, must wait 12 weeks before vaccination)
